Today I want to share a series of ultrasound images of an arm after being punctured. This case tells us that the ecchymosis and swelling may not merely be a hematoma.
The arm was quite swollen when I saw it. A solid mass could be palpated. There was a spheric scar on the top of the mass. It was said to be the puncture site.
Under the ultrasound image, a large cystic lesion could be clearly seen, which is about 22 mm x 14 mm x 13 mm in size. There seemed to be some cloudy, whirling content within. The cyst has a stalk connecting to an adjacent artery(brachial a.). The whirling turbulence was produced by the blood projected into the cyst. There is no obvious vessel wall around the cystic lesion.
Under the Doppler mode, blood flow projection from the artery into the cystic lesion and the turbulence within the cyst can be objected. Different colors refer to opposite flow direction, which mimic the shape of a Tai-chi symbol.

發生阿基里斯腱撕裂以後,球員即使立刻進行手術,要回到賽場往往也需要經過9~12個月的復健訓練,要回到較好的表現則可能要兩年以上。
雖然阿基里斯腱撕裂對職業選手影響這麼大。但在不同類型運動間,能回到賽場的球員比率也有不同,其中職業美式足球最低只有65.6%,職業籃球次之有68%,而職業棒球選手阿基里斯腱手術後的回場最為樂觀,根據統計100%可以回到賽場上(但因為統計人數太少,可能存在有誤差)。



當然我們也不用過度悲觀,這篇文獻最後把所有職業運動員的上場天數、上場時間、場上表現等因素一起統計,綜合評估起來發現,阿基里斯腱手術後,職業選手只要能夠撐過第一年,在第二年不論上場天數、上場時間、場上表現都有機會恢復到與其他選手相近的地步。
